  • Green Mountain Care Programs
    • Vermont Medicaid - Medicaid is low-cost or free health coverage for adults
      • Dr. Dynasaur - Dr. Dynasaur is Vermont’s low-cost or free health insurance for children, teenagers under age 19, and for pregnant women
      • Medicaid for the Aged, Blind, or Disabled - for individuals applying for Medicaid based on being age 65 or older, blind, or disabled.
      • Long-Term Care / Choices for Care - Choices for Care provides a package of long-term services and supports to Vermonters who are age 18 years and over and need nursing home level of care.  
    • Prescription Assistance - A program that helps Vermonters pay for prescription drugs

  • Children with Special Health Needs Program - Children with Special Health Needs (CSHN) supports Vermont children and youth with special health needs by ensuring comprehensive, culturally sensitive, community-based and family-centered services. This is a free public health program for families.
State Health Initiatives
  • Vermont Chronic Care Initiative - supports patient self-care, appropriate utilization of evidence-based health care services, and efficient coordination among providers, hospitals, community resources and state agencies. It is a holistic approach that considers physical, behavioral and socioeconomic barriers to health improvement, such as challenges with safe and affordable housing, food security, transportation to medical appointments and health literacy.
  • Blueprint for Health - The goal of the Blueprint is to provide the community with a  continuum of seamless, effective, and preventive health services, while reducing medical costs.
