The Agency of Human Services administers some programs and services to Vermonters locally, based out of twelve districts. Field Services Directors also work across internal programs to ensure that individuals and families have access to the range of services for which they are eligible. They also collaborate with community organizations in each district to ensure that our efforts to improve health and well-being are coordinated and aligned, leveraging and supporting local planning and action.

Vermont Team-Based Care is based on the concept of person-centered team-based approach to health incorporating both individual and system-level practices in support of individuals with chronic health conditions, mental health needs, substance use challenges, and social needs. Initially developed as Complex Care in 2015, VTBC combines healthcare and human service organizations working in partnership with individuals and families to identify their needs, link them to the right resources, and support the realization of their personal goals.
