Measurable Impact

Explore data about how Vermonters are doing and how AHS is contributing to improving outcomes

AHS believes that in order to effect systemic change and improve population health, communities, community organizations and service providers, business, government, and policy-makers need to have access to data that helps them understand how Vermonters are doing and how to make measurable impact.

Explore a prototype of our AHS Community Profiles of Health and Well-Being, where Vermont population trends are available by county, and in some instances AHS District and Health Service Area.  Compare trends over time, by geography, and in relation to state averages. Make sure to follow links to learn more about the “story behind the baseline” and see what strategies are underway across Vermont to improve quality of life.

While our AHS Community Profiles of Health and Well-being are under development, explore the prototype, which demonstrates trends related to health and well-being reported to the Vermont Legislature in Act 186.

In 1995, then Secretary of AHS Con Hogan established the AHS Community Profiles initiative, developed and produced annually by the Agency Planning Division.  The Profiles were published annually in a bound paper book and in PDF form online, distributed in communities, to service providers, and at the Legislature.  The profiles were last published in 2008.

The profiles were used to support data-driven planning at AHS, in communities, and in the state house, stimulate community-wide discussions about how to “turn the curve,” and to support data-driven grant applications.  Comprised of about 60 indicators, the profiles sought to quantify outcomes of well-being later established in statue in 1999.  The data provided reflected a partnership between AHS and the then Department of Education, encompassing key indicators for success in school, as well as foundational family, individual, and community conditions of health and well-being, by supervisory union.

What is a Scorecard?

The AHS Scorecards contain updated data and information about the well-being of Vermonters, as well as the quality and impact of AHS programs to improve outcomes for the people we work with. The Scorecard tool is designed according to the Results Based Accountability™ (RBA) framework, and helps us demonstrate our commitment to driving and supporting outcomes-oriented and data-driven strategies at the state and local level.

The public, the Legislature, community partners, and providers can use the AHS Scorecards to assess quality of life and well-being in Vermont, collaborate on strategies that will have measurable impact, and understand to what extent and how AHS programs are making a difference. 

In each Department and across AHS, we are building, maintaining, and using Scorecards to strengthen how we measure, monitor, and continuously improve our programs and organization with a focus on whether the individuals and families we work with are “better off.”

Vermont Medicaid’s Global Commitment to Health (GC) Core Measure Set
The Department runs and reports out on a variety of standard quality measure sets annually. Through regular review and analysis, a core set of these measures, the Global Commitment to Health (GC) Core Measure set, is chosen to be reported on by the DVHA to the Agency of Human Services (AHS). Performance on these measures is an indicator of overall Medicaid beneficiaries' health.

Outcomes: "i.e., All Vermonters are Healthy and Safe." 

According to Results Based Accountability, outcomes or results are ideal conditions of well-being for all Vermonters that groups of stakeholders and partners work together to achieve. At the Agency, we are guided by these outcomes for Vermonters in the work that we do across all of our programs, departments, and as One Agency. The Agency of Human Services Scorecard offers information on the four Results from the AHS Strategic Plan.


Indicators: "i.e., % of adults (20+) who are obese."

According to Results Based Accountability, indicators are measures for the whole population that help quantify whether we are making progress toward desired outcomes in Vermont. All partners and stakeholders in the private, public, and non-profit sectors, as well as in our communities share accountability for "turning the curve," or making an impact on these measures, as we work together to achieve whole-population outcomes for Vermonters. At the Agency, we work across Departments, across health and human services sectors, and across the life span to strengthen our contribution to improving indicators.  Each of the four AHS Strategic Plan results looks at a set of indicators to help us understand how Vermonters are prospering and where we need to focus our efforts.


Performance Measures: 'i.e., % of children 0-12 participating in the child care financial assistance program (CCFAP) enrolled in quality child care programs of 3,4,5 STARS."

According to Results Based Accountability, a performance measure is a measure of how well a strategy, program, division, department, agency, or service system is working to produce results for the people benefiting from the work. Performance measures help answer three basic questions so that we can continuously improve how we can best meet the needs of the people we serve: 


1) “How much did we do” (effort/quantity); 

2) “How well did we do it” (quality); 

3) “Is anyone better off” or “Did we make a difference” (impact)
