Leadership Team Meeting – AHS Burlington District

1. The Utilization of Service Coordination to better achieve outcomes
  • Began with a discussion of the definition of case management/service coordination and the steps involved in the process: assessment; identification of needs and goals; identification of resources; plan creation, implementation, reassessment.
  • Key principles: client-centered, outcome-oriented, respectful, strength-based, outcomes based, coordinated with other plans.
  • Other considerations: natural supports, faith community, formal and informal resources, eco-mapping: context of family in the community, therapeutic dose, service coordination within program vs. comprehensive service coordination.
  • Review of Current System across lifespan created by group. (attached)
  • Helpful to have a more comprehensive grid that would identify service, population served, eligibility criteria, discharge/transition criteria, service priorities and goals.
  • Reviewing the spectrum of services, the group next turned attention to the gaps.
    1. General assistance population-adults with disabilities unable to work, adults with mental health needs that aren’t CRT eligible; both may receive some services but not service coordination.
    2. Homeless families moving out of COTS - service coordination ends when they leave.
    3. Families with connection to the Children’s’ Mental Health System often include parental needs which are not addressed.
    4. Immigrant population not entitled to services; there is limited funding and limited services.
    5. Transition aged youth.
    6. Coordination is centered on a service as opposed to an individual or family.
    7. 211 currently has much of this information. We can work with them to put the data generated from this discussion into a grid.
  • ID Major Questions and Action Steps.
1.      How can we raise the bar in terms of services being accountable for not only providing & coordinating services but recognizing client needs and help that client move forward? How do we continue to support and empower client to succeed?
a.       Cross-training with a more comprehensive view.
b.      Build capability of clients not capacity of system.
c.       Tie service coordination to prevention to maximize resources.
 
2. Building Update
  • Contracts have been signed for both new sites; we are moving!!!!!!!
  • Renovation of the 101 Cherry St. site was slated to begin this week.
  • Field Services will be sharing two offices: one at Williston, one at 101 Cherry St.
  • Direct services will be concentrated in downtown Burlington campus and Winooski.
  • Map being created for all clients to locate service location when move is complete and will be available on the website.
 
3. Partner Updates
  • VT Refugee Resettlement Program announced the August 2nd Refugee Service Provider Meeting Agenda which will be reviewing the VRRP plan for next year. This includes an increase in the refugee population and a need for input from the community.
  • Children and Adult Mental Health Issues in Refugees is a training that will happen on August 23rd: live web-cast to be held in room 2B, 108 Cherry St. from 1:30-5pm. Two national speakers will attend and SW credit will be available.
  • Tana Randall (893-4878) announced the PCAVT needs volunteers to lead nurturing parent class. Stipend are available for teacher/volunteers.
  • ON November 2nd: the MFCN (Military Family Community Network) will be holding a full day event – “Strengthening the Community for Our Military Families” at Camp Johnson from 8:30 to 4pm.
 
4. Other
  • Peer Navigation Grant - Jane will follow up-211 for feedback
  • Survey Distribution – An email had been sent out from Dep. Commissioner Cindy Wolcott requesting feedback on a Differential Response in Child Welfare Plan that is being considered in Vermont. It will be recent to Leadership Team for their consideration.
  • Given multiple scheduling issues, the group rearranged meeting dates for the next few months. Please review the schedule below.
