What is service coordination?
Service coordination is the systemic process for coordinating informal supports and formal services across agencies, departments and the community. The goal of service coordination is an integrated delivery system that is responsive to the needs of the individuals and families served by AHS and the community. Service coordination identifies through a teamwork process the needs of the family or individual to create a coordinated plan that builds on the strengths and abilities of the individual and family.
Who should be referred?
Individuals and families can be considered for referral when there are multiple departments or organizations involved or the individual and family have complex and unmet needs. Referrals can also be considered when service goals of those departments or organizations run contrary to one another creating confusion or conflict. Service coordination is not a response to an acute crisis or emergency, but rather it is a deliberative planning process that can address future crises or emergencies.
What is the role of the service coordinator?
The service coordinator’s role is to assure that the teamwork approach is used to develop an integrated service plan, team members, including the individual or family, are engaged in the implementation of the plan and effective communication and information sharing takes place amongst team members. The service coordinator does not replace the direct service responsibilities of the involved organizations, but is intended to serve as a time-limited team facilitator to support providers, individuals and families.
How do I make a referral for service coordination?
Referrals can be made by e-mailing, faxing or sending a “service coordination referral form” to Don Mandelkorn, AHS Field Director. All referral requests will be acknowledged within two business days.

Peer Navigation
What is peer navigation?
Peer navigation is the delivery of information, referral and assistance for parents with disabilities themselves or with children who have disabilities. The Peer Navigator will assist people “navigate” the human services system partnering with them to become connected to those services and supports designed to meet their needs.
Who should be referred?
While the focus (based on the source of funding) is to support parents with disabilities the peer navigator can also support the parents and guardians of children who have disabilities through assistance with school related issues, referral to agencies and supports, and help with completing applications, appointments and making connections to services related t their disabilities.
What is the role of peer navigation?
The peer navigator can provide phone support for questions regarding human services that are available and applicable to the family’s situation, and will also meet with people to help support the parent’s efforts to build on their strengths and abilities by creating a family plan that addresses the needs and goals of the entire family, and to help find solutions together. When needed, the peer navigator will work with the service coordinators to bring service providers together with the family to foster solutions and communication.

You can contact the peer navigator directly, or leave a message at the toll free number below, which is the Vermont Federation of Families for Children’s Mental Health office. Please remember that this is a part time grant which creates a capacity concern from time to time.
